Region
Ten offers a choice of one Preferred Provider Option (PPO)
plan and two Health Maintenance Organization (HMO) plans through
Southern Health. All plans provide the same prescription
drug coverage. There is a one-month waiting period beginning
with the first of the next month after the date of hire. Example:
hire date of 7/18/05; coverage would start on 9/1/05.
If
you decline at initial enrollment, you may not apply again
until OPEN ENROLLMENT. Each employee has the option of deducting
their share of the premium in tax-free dollars under Section
125 of the Internal Revenue Tax Code.
Summaries
for our three plans will be made available on the website
shortly. Benefit summaries, physician directory and cost tables
for all three plans are available in the Human Resources office
located at 502 Old Lynchburg Rd.

Prescription Drug Plan. CareMark is a drug formulary,
or a listing of prescription medications, covered by Southern
Health health plans. This continually updated list includes
both generic and brand name prescription medications, and
all have been FDA approved. You are, of course, free to choose
(with your physician) any prescription medication, whether
it is on our list or not. While your prescription drug coverage
will determine your out-of-pocket costs, here are some general
guidelines for a standard three-tier plan:

Middle Copayment=Brand name drugs listed in our
formulary $20
Highest Copayment=Brand name drugs not listed
in our formulary $35

Dental Insurance |
Coverage
is effective after a waiting period of 3 full months beginning
with the first of the next month after the date of hire, i.e.,
coverage begins November 1 with a hire date of July 18. An
employee may choose between individual or family coverage.
Delta Dental of Virginia is the carrier for dental coverage.
Two plans are offered:

Low
Option: Provides 100% coverage for diagnostic and prevention
care such as oral exams, cleanings and fluoride treatments.
Basic dental care is coverage at 80% and includes fillings
and simply extractions. Maximum benefit per calendar year
is $1,000 per person |

High
Option: Provides the same coverage as the low option plan
with the addition of crowns, prosthodontics (partial or
complete dentures), and orthodontics for dependent children
to age 19. Maximum benefit per calendar year is $1,250
per person. Orthodontic lifetime maximum $1,000. |

Vision Insurance |
Coverage
is effective after a waiting period of 1 month beginning with
the first of the next month after the date of hire; i.e. coverage
would begin 9/1 for an employee with a hire date of 7/18.
Using Spectera’s provider network provides a member
with a complete examination as well as spectacle lenses and
frames, or contact lenses. Out-of-network benefits are provided
that pay a specific amount based on the service. Spectera
provides access to discounted laser eye surgery procedures
nationwide. Coverage is available for
Employee Only; Employee/Spouse; Employee/Child(ren) or
Employee/Family.
A
pamphlet explaining coverage and listing providers is available
in the Human Resources office at 502 Old Lynchburg Rd. The
provider network can be obtained 24 hours a day, 7 days a
week via Spectera’s web site, www.spectera.com. If you
