Ten operates clinics in each of the four rural counties we serve:
Greene, Fluvanna, Louisa, and Nelson. These clinics extend the
same breadth of services available in Charlottesville/Albemarle.
Those living in the rural areas we serve are also eligible for
any service in Charlottesville. Please call your local
clinic to learn more and to schedule an Intake Appointment.

Description |
| Unscheduled,
and in some instances scheduled services,
available 24 hours per day and seven days
per week to include assessment, crisis intervention,
hospital diversion, pre-admission screening
and other activities for the appropriate use
of institutionalization. Services may be provided
over the telephone or face-to-face and may
occur in clinics, detention centers, police
departments, hospital emergency rooms, or
other locations. Some of these services are
provided in association with the judicial
commitment process. In all cases attempts
are made to utilize the least restrictive
means to assist a consumer in crisis. |

mental retardation, or substance abuse issue. |

Description |
Psychosocial
Rehabilitation (PSR) is a set of service interventions designed to work holistically
to improve an individual’s functioning, the management
of his/her illness, and to facilitate recovery.
Specific areas of intervention addressed
by the PSR field include: daily
living skills; social network and supports; finances
and benefits; housing; education; employment;
legal status; transportation; and advocacy.
PSR
interventions occur concurrently with necessary
clinical treatments. The
goal is to accomplish and support the fullest
possible integration of the individual as an active
and productive member of his or her family, community
and/or culture.

| Population
Served |
Persons
(18 years or older) whose social and vocational
functioning is in need of training and supports
to achieve optimum independence and ongoing
stability in the community. Severity may
cause the person to be at risk of homelessness;
repetitive crisis interventions, psychiatric
hospitalizations, interventions by the criminal
justice system, or other institutionalization.
Potential recipients need intensive skill
development and supports to manage symptoms
and/or improve functioning in order to avert
these risks. Individuals must have a willingness
and desire for such services and
the ability to function within consumer/staff
ratios established by PSR organizational
structures. Individuals must also be free
of current, active chemical dependency.
| An
individual must meet one of the following |
| 1. |
Has
experienced a long-term or repeated
psychiatric hospitalization. |
| 2. |
Lacks
daily living skills and interpersonal
skills. |
| 3. |
Has
a limited or nonexistent support system. |
| 4. |
Is
unable to function or remain in the
community without intensive intervention,
or long-term services. |
|
| An
individual must meet two of the following |
| 1. |
Has
difficulty in establishing or maintaining
normal interpersonal relationships to such
a degree that he/she is at risk of hospitalization
or homelessness because of conflicts with
family or community. |
| 2. |
Requires
help in basic living skills such as maintaining
personal hygiene, preparing food, maintaining
adequate nutrition, or managing finances,
to such a degree that health or safety is
jeopardized. |
| 3. |
Exhibits
such inappropriate behavior that repeated
interventions by the mental health, social
services or judicial system are necessary. |
| 4. |
Exhibits
difficulty in cognitive ability such that
he/she is unable to recognize personal danger
or to recognize significantly inappropriate
social behavior. |
