Supported Living Service
In-home services provide intermittent supports to people living in their own home, apartment or with family. These services are designed to augment natural supports and are a less restrictive residential option. Services may take the form of monitoring medications, assisting with grocery shopping, health and medical appointment supports, etc. The in-home model is designed to preserve independence while keeping people healthy and safe. Our person-centered services strive to offer the greatest opportunities for unrestricted participation and community inclusion. Training may be provided in personal care, domestic skills, leisure time training, daily life skills and social skills. Individual served are referred by their Region Ten ID case manager.
