Case Manager/ Developmental Disability

Seeking a motivated individual to join a team of compassionate professionals. Direct Support Professional III/ Meadowcreek, Day Support

Direct Support Professional I , Residential

Join our team supporting adults with developmental disabilities in a group home setting.  Region Ten CSB is committed to supporting adults with developmental disabilities in experiencing a healthy, safe, and meaningful life at home and as part of the community.  Direct Support Professionals (DSP)s are responsible for maintaining a safe and supportive environment, fostering growth and independence, and helping each person reach his/her individual goals.  The Direct Support Professional (DSP) is also responsible for providing assistance in daily living skills, providing meals and nutrition, upkeep/ maintenance of the residence, medication management, and transportation for individuals with developmental disabilities. Ability to use Electronic Health Record to record extensive and detailed documentation, handle physical aspects of job, including bending, stooping, lifting, pushing, pulling, reaching, and walking for periods of time, and medication administration as required by state/program requirements a preferred candidate will possess. Clinician I, Housing Peer

Region Ten is seeking to fill a position of Clinician who is responsible for developing and implementing self care skills training programs concerning individual clients (health and safety, personal care, activities of daily living, use of community resources).  The incumbent will also assist clients with medication management, monitor health, nutrition and physical condition, provide assistance with personal care, ADLS and use of community resources. The Clinician will work in collaboration with the Permanent Supportive Housing Team to ensure housing stability of program participants.  Must meet qualifications as QMHPA (Qualified Mental Health Professional-Adult) or QMHP-E (Qualified Mental Health Professional-Eligible) as set forth by Department of Behavioral Health and Developmental Services. Residential Care Coordinator, Mohr Center

The Residential Care Coordinator serves as a Substance Abuse Case Manager is responsible for providing and extending triage, assessment, emotional support and access to recovery support services to individuals receiving services at the Mohr Center who have a history of opioids abuse or dependence.  Will provide care coordination and medical monitoring for individuals engaged in the program and collaborate with individuals at Region Ten as well as external partners providing treatment to individuals in the program.  Associates degree required. Bachelor’s degree or Certified Peer Recovery Specialist preferred. Two years of experience in a program directly concerned with Substance Abuse treatment, co-occurring treatment or rehabilitation is preferred.   A valid Driver’s License plus an acceptable driving record as issued by the Department of Motor Vehicles are required. Clinician II/III – Mental Health & Substance Abuse, Louisa

Clinician with experience and expertise in providing assessment and treatment services to those with Mental Health and recovering from Subtance Abuse. Provide outpatient assessment and ongoing mental health and substance abuse counseling services to adults and adolescents; treatment modalities to include individual, couple, family and group counseling and the use of acupuncture. The incumbent is expected to develop and present substance abuse education and treatment curriculum for clients referred for a 10-week treatment program by James River ASAP and to accurately complete required reports.  Additionally, the incumbent provides evaluation for adolescents as referred by the schools and courts.
