Emergency Services and Case Management

The primary focus of the Center for Access and Case Management is to provide a central point of entry to services for persons with addiction, mental health, and intellectual disability needs. As the front door to the agency, we work to assure that individuals served have access to appropriate services.


Emergency

Program of Services: Crisis services are available 24 hours a day, 7 days a week. The services include counseling and phone consultations with or for people in crisis, as well as admission screenings for Region Ten's crisis stabilization center, psychiatric hospitalizations, and/or substance abuse inpatient treatment. Services may be provided over the telephone or face-to-face and may occur in clinics, detention centers, police departments, hospital emergency rooms or other locations. Some of these services may be provided in association with the judicial commitment process. In all cases, the least restrictive means to help a consumer in crisis is used.

Individuals served: Emergency Services serves all individuals, of any age, ethnicity or disability, who are in crisis due to mental illness or substance abuse.

Program Hours of Operation: Crisis services are available 24 hours a day, 7 days a week.

Payment for Services: This service accepts Medicaid Insurance as well as self- pay from the consumer. Sliding fee scale may be available for qualifying individuals.

Medical

Program Description: An array of center-based medical and associated nursing services that focus upon pharmacological evaluation, treatment, management, and monitoring of DSM IV disorders. This service is provided in conjunction with case management/care coordination and is not a stand-alone service.

Specific services include psychiatric evaluation and assessment; psychiatric medication management; laboratory and ancillary services; provision of medication education and informed consent; and dispensing of medications and other pharmacy related activities.

Population Served: Outpatient medical services are appropriately provided to adults having confirmed or suspected DSM IV disorders thought to be in need of pharmacological evaluation, treatment, and/or monitoring and who choose and desire to receive such services.

Access to Program: Appointments may be scheduled through a case manager or care coordinator.

Payment for Services: This service accepts Medicaid Insurance as well as self- pay from the consumer. Sliding fee scale may be available for qualifying individuals.

Adult Case Management - Intellectual Disabilities

Program Description: Services are offered that assist individuals with intellectual disabilities in accessing the full range of needed supports to assist them in their daily lives.

Individuals Served: Persons who have a documented diagnosis of an intellectual disability as defined by the Virginia Department of Medical Assistance Services (DMAS, and who desire assistance with with monitoring, linking and coordinating the services they need.

Program Hours of Operation: This service operates Monday- Friday, 8:30 a.m. - 5:00 p.m.

Evidence Based Information: This service adheres to the guidelines provided by the Virginia Department of Behavioral Health and Developmental Services (DBHDS).

Access to Program: Call (434) 972-1800 to schedule an appointment.

Adult Case Management - Serious Mental Illness

Wellness Recovery Center

Program Description: The Wellness Recovery Center is an eleven bed, short term, residential crisis stabilization program for adults experiencing an acute mental health crisis. Program is staffed by a multi-disciplinary team of professionals who provide person-centered, recovery based support in a safe, comfortable, peaceful home-like environment.

Individuals Served: Adults experiencing a mental health crisis that may put them at risk of hospitalization, becoming homeless, or becoming isolated from social supports.

Program Hours of Operation: This 24 hour program operates continually year round.

Evidence Based Information: Skill development related to maintaining personal safety, recovery, understanding the influence of substance use on mental health, and Dialectical Behavioral Therapy (DBT) skills including mindfulness,emotional regulation, distress tolerance, and interpersonal effectiveness.

Access to Program: Please call the 24-Hour Crisis Number at (434) 972-1800. The Community Services Board Emergency Services through out the region will screen for admission to the Wellness Recovery Center.

Payment for Services: Funding sources include Medicaid and self-pay. Sliding fee schedule is available for self-pay.

Hospital Liaison Services

Program Description:Transition services are available to assist individuals returning to their community. Staff work collaboratively with hospital treatment teams, community providers and family members to ensure a successful transition back to the community.

Individuals Served: Persons 18 years of age and older from the Region Ten area (Charlottesville city, Albemarle, Greene, Louisa, Fluvanna, Nelson counties) who are hospitalized in a state psychiatric facility needing liaison services are eligible.

Program Hours of Operation: This service operates Monday - Friday, 8:30 a.m. - 5:00 p.m.

Evidence Based Information: This service adheres to the guidelines provided by the Virginia Department of Behavioral Health and Developmental Services (DBHDS).

Access to Program: Referrals to this service are made through state psychiatric hospitals.

Payment for Services: This services is covered by state funds allocated to the Community Services Board.
