Sponsored Placement (Sponsored Residential Services)
Service Overview
Sponsored Placement/Sponsored Residential offers a continuous (up to 24 hours per day) service. Up to two individuals live in the home of a professional service provider and become an integrated part of the family. The sponsored providers provide most of the needed services. They may also hire other trained and certified workers to assist them with providing support. In this service, the goal is to help each individual with developmental disabilities find a mutual match with a sponsor family. The intent of this service is to offer personalized services that will address the unique needs of each person so that the natural supports of a family, coupled with the support of Region Ten, will allow the person to live the life they desire in the community.

Can I make a referral directly to sponsored residential services? If you have DD Waiver funding or are assisting someone with DD Waiver to explore services, please reach out to your case manager/support coordinator to inform them of your interest in sponsored residential services. What services are offered in sponsored placement? Services are based on the Person-Centered Plan (PCP) of each individual enrolled in this service. The PCP is also known as the Individualized Service Plan. This plan is developed with the Case Manager/Support Coordinator, the individual, service providers, and other people chosen by the individual. Each plan is strength-based and includes what is important to the person as well as what is important for the person to be healthy and safe. These services consist of skill-building, routine supports, general supports, and safety supports that enable an individual to acquire, retain or improve the self-help, socialization and adaptive skills necessary to reside successfully in home and community–based settings.
