Meadowcreek Center is a community integration program that provides training, support, and supervision to adults with developmental disabilities. The program aims to help participants acquire, retain and/or improve their self-help, social and adaptive skills through person-centered planning, behavioral support, and assistive technologies.

Independent Living Supports
Support for individuals with developmental disabilities to achieve and maintain a life based on person-centered choices, interests, and preferences
Community Housing (Group Home Residential)
Licensed group home setting to achieve and maintain a full life based on person-centered choices, interests and preferences
Sponsored Placement (Sponsored Residential Services)
Up to two individuals live in the home of a professional service provider and become an integrated part of the family
In-Home Support Services
Designed to ensure the health, safety, and welfare of individuals with developmental disabilities
Day Program
A safe and supportive environment that promotes independent living for those recovering from mental illness
Supported Employment
Supported work group for individuals with developmental disabilities to share a work position with onsite staff supports
Community Engagement
Support and opportunities for individuals with developmental disabilities to pursue their personal interests in the local community
Meadowcreek Day Support
Community program that provides weekday skill-building and support to adults with developmental disabilities
